OPEN HEART SURGERY UNDER EXTRACORPOREAL BYPASS COMBINED WITH MODERATE HYPOTHERMIA AND 5% DEXTROSE PRIMING

Abstract
The utilization of a moderate hyphthermia and 5% dextrose priming in conjunction with extracorporeal bypass for open heart surgery has not only simplified the procedure but also has offered several advantages which are not available when these are not used. Among the advantages are: 1. Teh heart lung machine could be made simple and compact. 2. The unit is more ecomical to operate in terms of personnel and equipment. 3. it eliminates the nees for a large amount of homologous, heparinized blood. 4 It prevents hematological complications including minor incompatibility, hepatitis and thrombocytopenia. 5. It increases the availability of the pump-oxygenator as a standby unit.
A clinical trial with this method using the pump oxygenator system as developed by Zuhdi has proved to be satisfactory and the results are reported. In our inital series of 10 consecutive case, there were three operative deaths. These were in two cases of tetralogies of Fallot and a case of mitral stenosis in whom open corrctions were attempted. In none of these cases, the pump-oxygenator system was considered to be the direct cause of death.
The surviving patients were atrial septal defect, 4;rupted aneurysm of the aortic sinus, 2; and ventricular septal defect, 1. All of the surviving patients obtained an excellent result.
